Aspire Health Partners seeks a CVN Licensed Therapist to provide outpatient counseling and recovery-focused services for adults, children, and families affected by mental health and substance use disorders. In this role, you will complete diagnostic assessments, develop person-centered treatment plans, and deliver individual and group therapy using evidence-based, trauma-informed approaches. You will collaborate with a multidisciplinary team, coordinate community resources, and maintain timely clinical documentation. Responsibilities
- Provide individual, family, and group therapy for clients with mental health and substance use disorders
- Conduct biopsychosocial and diagnostic assessments and develop person-centered treatment plans
- Deliver evidence-based, trauma-informed interventions to support recovery and resilience
- Coordinate care with psychiatrists, nurses, case managers, and community partners
- Respond to crisis situations, conduct risk assessments, and implement safety plans
- Maintain accurate, timely clinical documentation in the electronic health record
- Engage clients and families in recovery planning and education about mental health and substance use
- Participate in multidisciplinary team meetings, case consultations, and quality improvement activities
