Social Services Director
Mission Valley Post Acute Full-Time | $85,000 per year + Benefits Position Summary
Mission Valley Post Acute is seeking a compassionate, organized, and dedicated
Social Services Director to join our team. The Social Services Director will provide medically related social services to residents and their families, helping each resident achieve and maintain the highest practicable level of physical, mental, psychosocial well-being, independence, and quality of life.
The ideal candidate is an empathetic and dependable professional who can build trusting relationships with residents and families while effectively coordinating care with our interdisciplinary team and community partners.
Key Responsibilities
- Assess residents' social, emotional, psychological, and family needs.
- Develop and implement individualized social services care plans.
- Provide counseling, emotional support, and crisis intervention to residents and families.
- Assist residents and families in coping with illness, disability, loss, grief, and changes in independence.
- Support residents with healthcare decisions and advance care planning.
- Assist residents in achieving the highest practicable level of self-care, independence, and well-being.
- Provide discharge planning and coordinate services, equipment, referrals, and community resources.
- Connect residents with appropriate community, social service, health, and welfare agencies.
- Coordinate outpatient appointments and on-site ancillary services, including optometry, podiatry, dentistry, and psychiatric services.
- Assist residents in obtaining prosthetics, assistive devices, and other needed resources.
- Maintain accurate and timely documentation regarding residents' social services status and needs.
- Develop strong working relationships with nursing, rehabilitation, dietary, activities, administration, and other departments.
- Maintain relationships with outside agencies and community organizations.
- Assist residents with personal shopping and other appropriate personal needs.
- Assist with inventory and tracking of resident belongings.
- Coordinate responses to reports of missing, lost, or stolen belongings.
- Provide staff in-service training as requested.
- Participate in interdisciplinary care planning and resident/family meetings.
- Advocate for residents' rights, preferences, dignity, and quality of life.
- Maintain confidentiality and comply with applicable healthcare regulations and facility policies.
Qualifications
Required
- Bachelor's degree in Social Work, Human Services, or a related field .
- Minimum of 2 years of supervised social work experience in a healthcare setting , working directly with individuals.
- Strong communication, organization, planning, documentation, and interpersonal skills.

This position has
no direct supervisory responsibilities .
Compensation & Benefits
Salary: $85,000 per year Full-time employees are eligible for a comprehensive benefits package. Benefits may include healthcare coverage, paid time off, retirement benefits, and other employee benefits.
