About the Role

As a Discharge Planner, you will play a vital role in supporting clients as they transition from inpatient care to the next stage of their recovery journey. Working closely with an interdisciplinary treatment team, you will coordinate discharge services, connect clients with community resources, and help ensure continuity of care for long-term success.

Key Responsibilities

  • Coordinate discharge planning and follow-up care services
  • Collaborate with clinical teams to support treatment and recovery goals
  • Conduct family meetings and gather collateral information
  • Participate in multidisciplinary treatment team meetings
  • Facilitate patient engagement and support therapeutic programming
  • Connect clients with community resources and aftercare services
  • Maintain accurate and timely documentation

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field
  • Recent graduates are encouraged to apply
  • Experience in discharge planning, case management, behavioral health, or related services is preferred
  • Strong communication, organizational, and collaboration skills
  • Passion for helping individuals achieve lasting recovery and wellness
