Join Our Team as a Social Services Director!McKinley Park Care Center is seeking a
dynamic, compassionate, and organized Social Services Director to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about supporting residents and their families through every stage of care.
What You'll Do- Develop individualized care plans by working closely with residents and their families
- Support residents in achieving the highest level of independence and well-being
- Provide medically related social services to enhance residents' physical, mental, and psychosocial health
- Assess emotional, psychological, and social needs of residents
- Offer counseling and emotional support to residents and families
- Assist with healthcare decision-making and coping with illness, disability, or loss
- Coordinate discharge planning with interdisciplinary teams and outside agencies
- Connect residents with community resources and support services
- Schedule outpatient appointments and coordinate ancillary services (optometry, podiatry, dental, psychiatric, etc.)
- Maintain accurate and timely documentation
- Manage resident belongings and respond to lost/missing item reports
- Provide staff in-service training as needed
- Build strong relationships with internal teams and external partners
What We're Looking For- Bachelor's Degree in Social Work or Human Services required
- Minimum of 2 years of supervised social work experience in a healthcare setting
- MSW preferred
- Strong communication, organization, and problem-solving skills
- Ability to handle sensitive situations with professionalism and confidentiality
- A proactive, compassionate approach to resident care
