Job Description

*Licensed Clinical Social Worker*

*Title: *LCSW

*Reports To: *Chief Strategy Officer

*Supervision Exercised:* None

*Works Directly With:* Clinic Managers and the Primary Care Team

*Typical Working Conditions: *Work is performed in an office setting. There will be contact with clinic staff and outside organizations. No heavy lifting required. Some travel required in the Cortland/Cayuga County Region.

*Minimum Qualifications: *Masters Prepared Social Worker, Community Counselor, or Psychologist. Preferred at least 2 years relevant experience.

*Primary Function: Under the direction of the Chief Medical Officer and as a part of the integrated multidisciplinary primary care team, t*he Mental Health Therapist will provide direct evidence-based psychotherapy; including individual, couple, group and family counseling and services to patients (adults/youths); will include conducting comprehensive assessments, diagnosis, treatment plans and self-management plans. Additional referrals can be made by outside sources including self referral, Psychiatric specialists and family members.

*General Summary of Duties:*

* Provides evidence-based individual, group and family counseling and services with a high volume of patients and provide recommendations/support to the primary care provider or team.
* Conducts comprehensive assessments and diagnosis; develop treatment plans and aftercare planning.
* Coaches patients on how to manage ongoing health conditions and provides therapy to help patients make a positive behavior changes such as (assisting patients in developing a Self-Management Plan.)
* Counsels and provides crisis intervention and referral for patients, including assessment and treatment of emotional and behavioral conditions.
* The MHT will be expected to maintain a productivity level of at least 7-9 visits per day.
* Communicates and collaborates regularly with primary care team to address and tailor interventions/treatments to improve patients’ physical and emotional well-being.
* Work in collaboration with the primary care multidisciplinary team members including external community partners (refers appropriate cases for educational, employment and ancillary services.)
* Develops and researches preventative behavioral health, educational materials and resources for clinic team, families and patient needs.
* Communicates and provide consultation to the Family Health Network medical providers on behavioral health aspects of patient care.
* Participate in clinical supervision, case conferences and department meetings.
* Participate in Family Health Network’s Quality Improvement program, as requested.
* Consults with the Chief Medical Officer in creating and updating policies related to behavioral health.
* Provide and coordinate education and trainings as requested by the CMO and/or Leadership team to increase the primary care team’s understanding of behavioral health and its impact on physical health and wellbeing: (informal and formal trainings, making recommendations regarding screening methods, screening results, evidence-based treatments, etc.)
* Document warm hand off from Medical Providers in patient’s EHR.
* Document counseling sessions and care plans in a comprehensive and timely manner in the electronic health record (progress notes, SOAP Notes format).
* Exercise independent judgment and make appropriate decisions in the routine conduct of business.
* Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner.
* This position requires compliance with Health Center’s written standards, including its Standards of Conduct and policies and procedures pertaining to all operational aspects, furthermore this position is required to report any suspected instances of noncompliance.
* This position will function as a contributing member of the emergency preparedness team under the incident command system.
* Perform other tasks as assigned.
* Maintain strictest confidentiality.
