Clinical Coordinator I
Job Description: Clinical Coordinator II
The Clinical Coordinator I will supervise the provision of professional social work services, including but not limited to, supervision of social workers, case managers, recreation, medication and other program staff members, while carrying a small caseload. The Clinical Coordinator I reports to the Managing Director or Director of Clinical and Social Services and works closely with the Building Manager.
Responsibilities Provide clinical and case management supervision to all social work and case management staffProvide access to clinical supervision to all staff as neededAs a member of the Intake Committee, review all applications for admission, participate in home visits to prospective applicants and participate in all admission decisionsProvide clinical support and resources for any psychiatric emergenciesReview and sign support plans, case management summaries, case notes and psychosocial historiesEnsure compliance with contract and regulatory requirements thru regular chart audits to review completeness, timeliness and quality of documentationMeet regularly with Building Manager to review tenant and building issuesCoordinate with Building Manager on staff training and provide resident related counseling to building staff as neededLead a weekly team meeting with social work staffFacilitate monthly or quarterly meetings with Building Manager and social work staffProvide orientation and training for new social work staffReview incidents and incident reports and submit to contracted agencies as requiredAssume leadership role during site audits by contract agencies; write and submit any required Plans of CorrectionSupervise social work intern/s as neededClinical Coordinator II will develop a 1:1 relationship with residentsAssist residents in obtaining and maintaining appropriate medical and mental health servicesAssist residents in obtaining and maintaining entitlementsAssist residents in establishing and/or maintaining family and social contactsAssist residents in identifying socialization activitiesComplete required forms and reports for resident discharges and/or transfersProvide crisis intervention servicesVisit residents in hospitals, nursing homes or other care facilities as neededFacilitate community building through various modalities including group work services, activities and holiday celebrations Other responsibilities as assigned
Qualifications MSW required; LMSW preferredMinimum of 3+ years experience working with the older adults and/or people with a history of homelessness, mental health issues or substance use requiredSupervisory experience preferred
