Job Description

MDS Nurse (LVN/LPN) - PRN

Location: Canyon Springs Post Acute - San Jose, CA
Position Type: PRN (As Needed)
Pay Range: $45-$47 per hour

Position Summary
  • Coordinate and complete MDS assessments to ensure compliance with federal and state regulations
  • Maintain accurate clinical documentation to support reimbursement and care planning
  • Collaborate with interdisciplinary teams to improve resident outcomes


Key Responsibilities
  • Complete MDS/CAA assessments on time, ensure compliance, and submit for reimbursement
  • Collaborate on care plans, monitor resident progress, and participate in care conferences
  • Support audits, train staff on documentation, and stay current on regulatory updates


Qualifications
  • Active California LVN/LPN license and experience in skilled nursing or long-term care preferred
  • Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines
  • Strong organizational, teamwork, and EHR/Microsoft Office skills


Skills & Requirements
  • Effective communication skills with residents, families, and care teams
  • Ability to interpret policies, procedures, and clinical documentation requirements
  • Basic math, problem-solving, and attention to detail
  • License Required: Active LVN/LPN (California)
