The Case Manager – Bone Health & Fracture Prevention Clinic is responsible for coordinating comprehensive, evidence-based patient care services for patients identified as at risk for osteoporosis, fragility fractures, and bone health complications. This role supports continuity of care, regulatory compliance, patient education, and interdisciplinary collaboration to improve clinical outcomes, reduce readmissions, and promote quality-driven, cost-effective healthcare delivery.
The Case Manager serves as a clinical liaison between providers, patients, caregivers, diagnostic services, and community resources to ensure patients receive timely evaluations, treatment coordination, follow-up care, and preventative interventions aligned with Joint Commission standards, CMS quality expectations, patient safety initiatives, and evidence-based orthopedic and population health practices.
