Location/Department: Hospice
Part time - 24 hours per pay period
Job Summary:
Provide care/activities related to the physical, psychosocial, emotional, and spiritual needs related to the terminal illness to promote the patient’s well-being, comfort, and dignity throughout the dying process, as well as support for the patients’ families and Hospice team members.
Supervisory Responsibilities: None
Duties/Responsibilities:
- Demonstrates competencies for all populations served
- Assesses patient/family spiritual needs
- Provides on-going spiritual support/guidance to patients & families
- May conduct or make arrangements for funerals & memorial services
- Knowledgeable of grieving process & psychosocial ramifications
- Coordinates with other pastors
- Participates in IDG
- Coordinates with other disciplines
- Submits documentation timely according to departmental requirements
- Submits accurate documentation that reflects the actual day, time, & services provided
- All other duties as assigned
Required Skills/Abilities:
- Strong verbal, written, & listening communication skills
- Must be able to operate a computer, copier, phone, & automobile
