Required License/Registration/Certification Licensed Practical Nurse (LPN) – Kentucky Licensure Physical Requirements Requires handling of average weights or objects up to 25 pounds with some sitting, standing, or walking. Shift Day shift, 8:00am – 4:30pm, this is a hybrid position. Job Summary This LPN position is a member of the multidisciplinary ambulatory and Transitional Care Management team who is responsible for patient care, patient satisfaction, and clinical outcomes. As an LPN Nurse, you will play a vital role in supporting patients during transitions between healthcare settings, ensuring continuity of care and promoting optimal health outcomes. Skills / Knowledge / Abilities Care coordination, excellent communication and time management skills, health education teaching skills, and triage experience is a plus.
