We are a growing Home Health Company based in Farmington, NM and are expanding and are seeking an Administrative RN/Case Manager to work full time to aid in the growth of our growing census of bedside patients in the Farmington area.

The Administrative RN/Case Manager reports directly to the Administrative Director.ve Director.

*Duties*

As an Administrative RN/Clinical Service Manager, responsibilities include but are not limited to:

* Responsible for all day to day operations of Positive Nature Homecare.
* Collaborates with other Clinical Services Managers to coordinate and oversee all direct and indirect patient care and service provide by Positive Nature Homecare.
* Ensures organizational compliance with state, federal and local laws and regulations.
* Ensures staff compliance with state, federal and local laws and regulations.
* Develops, maintains, updates and implements written policies and procedures for community services entities, incorporates up to date evidence-based research into policies and procedures.
* Maintains communication with PNHC management and department heads through interdepartmental planning and collaboration.
* Participates in the recruitment and selection of personnel.
* Confers with staff members to discuss issues, coordinate activities and resolve problems.
* Assures systems are in place to implement corrective actions and plans to resolve community service problems and deficiencies.
* Demonstrates knowledge and application of quality standards and proactively monitors and implements systems to achieve excellence through QAPI programs.
* Ensures that a clinical service manager or designee is available during all operating hours.
* Works with and supports managers during licensing surveys and related correction processes.
* Serves as liaison between the licensee, personnel, contractors.
* Coordinates patient care services and oversees scheduling procedures.
* Manages patient records accessibility to ensure employees and contractor staff have necessary access including health care directives.
* Develops, implements, and evaluates methods to ensure effective and efficient use of staff, facilities, equipment, management, utilization review, and discharge planning programs.
* Develops and implements the treatment plan for each patient under the direction of the patient's physician.
* Consults with case managers and medical staff on patient care or nursing problems and interpretation of center policies to ensure patient needs are met.
* May perform patient bedside nursing duties as necessary.
* Supports and executes the mission, ethics, and goals of the company effectively. Represents themselves in a positive and professional manner in the company and community. Adheres to dress code with a clean and neat professional appearance. Reports on time and as scheduled in order to complete work within a designated time. Adheres to call company policies and procedures outline in Employee Handbook, Employee Agreement, or communicated from the executive team.

*REQUIREMENTS*

*Qualifications:*

· Bachelor’s degree in nursing from an approved institution (2 years minimum experience) required, Masters Degree preferred

· Current RN license and BLS certification

-Navajo speaker required

-EEOICPA program Experience required

· Valid driver’s license and proof of vehicle insurance

· Current TB test and fingerprint clearance card for NM

· Background check required

Job Type: Full-Time

Salary: $125,000 - $150,000 with $5000 Sign on Bonus after 90 days

Schedule:

* Monday through Friday 830am-430pm with ability to work on call 7 days a week as needed

Experience:

· Home Health: 1 year supervisory experience required

Work Location:

* Office with ability to travel to patient’s homes. May be required to take work as a fill in for sickness/vacation/emergencies of clinical staff as required.
