Physical Therapist – Full Time
Alpine Rehabilitation & Nursing Center– Ohio
Alpine Rehabilitation & Nursing is seeking a compassionate and motivated Full-Time Physical Therapist to join our growing rehabilitation department. Our therapists are part of a supportive interdisciplinary team focused on restoring function, improving quality of life, and helping residents reach their highest level of independence.
Position Summary
The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, delivering skilled interventions, collaborating with nursing and therapy disciplines, and ensuring high-quality clinical outcomes in accordance with state practice act regulations.
Responsibilities
Perform comprehensive PT evaluations and re-evaluations
Develop and implement individualized treatment plans
Deliver evidence-based PT interventions
Participate in discharge planning and recommendations
Provide family/caregiver and staff training as appropriate
Accurately complete documentation per facility, payer and regulatory guidelines
Collaborate with nursing, OT, ST, and interdisciplinary team members
Contribute to quality improvement initiatives and clinical excellence
Qualifications
Graduate of an accredited Physical Therapy program
Current Ohio Physical Therapist license (or eligible)
Prior SNF or geriatric experience preferred, but not required
Strong clinical reasoning and communication skills
Ability to work collaboratively in a team environment
