Assertive Community Treatment (ACT) is a client-centered, recovery oriented mental health service delivery model. The ACT team Psychiatric Nurse is a member of the ACT multidisciplinary team and is responsible for the following: Providing management and administration of medication in conjunction with the psychiatrist. The ACT Team nurse is also responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, providing a range of treatments including psychosocial rehabilitation. Will also provide support services to the ACT Team.
