Providence Home Health and Hospice is seeking a Full-time Physical Therapist. This job requires the Therapist to plan and administer prescribed skilled physical therapy treatment/training for our clients with various post-op procedures, injuries, illnesses and/or functional disabilities. You will evaluate the patient's physical strengths in order to develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team, along with the patient's personal goals established in order to establish measurable training and achieve maximum results. The Physical Therapist will provide the PTAs and Home Health Aide staff with written instructions/care plan that reflects current plan of care related to therapy.
