*Job Summary: * We are seeking a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) to join our care team. The clinician will provide counseling, therapy, and support services to patients across diverse backgrounds. This role involves conducting assessments, developing treatment plans, and working collaboratively with medical providers to ensure patients receive comprehensive, integrated care. The LCSW or LPC functions as an integral member of a team delivering a variety of services to patients referred by a primary care provider.
