Advanced Care at Home Physician -Nemours Children's Heath - Home Office Nemours Virtual Care Nemours is searching for an Advanced Care at Home physician to deliver acute, hospital-level medical care directly to a patient's residence using a blend of virtual technology, remote monitoring, and in-person visits. Nemours Virtual Care is a centralized, enterprise-wide model that delivers technology-enabled clinical services to support patient safety, care coordination, access, capacity optimization and acute care delivery across the health system. Core Responsibilities Patient Acquisition: Review and accept new hospital admissions or patient transfers into the home care program. Daily Rounding: Conduct virtual or in-person rounds to monitor recovery, update treatment plans, and evaluate vital signs.
