This position could be full-time or part-time (20 - 29 hours per week). Position would be working with Aging & In-Home Services and PACE of Northeast Indiana. *Summary/Objective* Optimizes nutritional status of PACE participants through comprehensive nutrition assessment, monitoring, and communicating concerns & recommendations to the PACE Interdisciplinary Team. Provide comprehensive nutrition assessments and counseling to patients at the AIHS GUIDE clinic. *Essential Duties/Responsibilities* · Perform nutritional assessments upon enrollment and re-assess every 6 months or as needed for significant changes in clinical status. Record periodic nutritional care data in the medical record. Review and adjust therapeutic diets to reflect any changes in the daily menu and suggest the use of supplemental feedings when necessary.
