*Position Summary* Under the general direction of the Center for Medication Management(s) Manager, provides ambulatory clinical pharmacy care promoting the safe and effective use of drug therapy, according to standards established by the Department of Clinical Pharmacy and guideline supported, evidence-based recommendations to physicians of and patients requiring anticoagulant therapy, diabetes management, transitional care disease state management, vaccinations, preventative wellness services, or referred medication management consults.
