JOB RESPONSIBILITIES: Provide and document patient/family counseling services. Assess patient’s circumstances to provide data on strengths and weaknesses of their social, emotional, functional and environmental situations and assist other disciplines in the same as a consultant and educator.          Assess and evaluate psychosocial and financial parameters for those "high risk" patients identified by but not limited to: Patient displays self-destructive behaviors. Patient has a documented history of psychological problems or developmental disability and may need long term care for services.
