Position Summary: Responsible as a member of the interdisciplinary team to the Administrator for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families consistent with agency policies and procedures. Will provide skilled nursing, intervention, and supportive care in patients' homes for weekend, holiday, evening, and night on-call duty in accordance with the Interdisciplinary Team plan of care for each patient.
