The LCSW-Social Worker for Case Management assists patients who present with complex, psychosocial needs and work collaboratively with the Case Management Team, Health Care Providers, and Physicians. Intervenes to help the patient and/or family understand and cope with personal, emotional and biopsychosocial issues affecting hospitalization and the delivery of care.
