Description POSITION SUMMARY The SDOH coordinator will implement and coordinate SDOH interventions across various organizational entities and in partnership with external stakeholders. This position supports program administrative functions, participant engagement, patient/community outreach and data collection and reporting. The SDOH coordinator will also serve as a member of the care team, working within the community setting, providing outreach to patients with medical and social needs.
