Position Title: MAT Care Manager - Physician's Clinic Location: Owensboro, KY | Beaver Dam, KY Description: Job Summary: The MAT Case Manager coordinates and supports patients receiving medication-assisted treatment for substance use disorders. They act as a liaison between patients, prescribers, nursing staff, and community resources to ensure continuity of care and promote long-term recovery. Essential Functions: Conduct intake assessments and develop individualized treatment and recovery plans. Monitor patient progress, adherence to prescribed medications, and overall treatment goals.
