This is a unique opportunity to work within the Neurology Division in a dual-role capacity, providing both therapeutic services and case management support. The position will be split evenly, with approximately half of the time dedicated to social work services for patients within our Cognitive and Comprehensive Neurology divisions, including care coordination, resource navigation, and case management.
