Nexus-Woodbourne is seeking two (2) Clinical Case Managers to join our Treatment Foster Care program , where you will provide therapeutic support and case coordination for youth with emotional and behavioral challenges in foster care. This role involves developing individualized treatment plans, collaborating with foster families and service providers, and ensuring timely access to mental health and social services.
