The Supervisor - Transition to Community Living (TCL) Care Management provides supervision and oversight to the TCL Care Managers who support a specified population of members utilizing behavioral health and/or physical health services within Alliance Health Plan. The Supervisor-TCL Care Management is responsible for ensuring the TCL Care Mangers work within the principles of the permanent supportive housing model to stabilize tenancy to coordinate services that attend to the whole-person health in support of members living integrated lives as valued members of the community.  This is a full-time hybrid opportunity.
