St. Camillus is seeking an experienced Hospice Clinical Care Manager to coordinate the day-to-day operations of our Hospice Agency and help ensure the delivery of quality, comprehensive care in accordance with federal, state, Joint Commission, and St. Camillus guidelines.
In this role you will:
Lead, coach, and support Hospice clinical staff.
Coordinate admissions, daily nursing visits, case conferences, and care services.
Monitor quality outcomes, documentation, compliance, and reporting practices.
