Position Overview
St. Camillus is seeking a compassionate and skilled Social Worker (CSW) to join our Assisted Living team. This role plays a vital part in supporting residents and their families through transitions, care planning, advocacy, and ensuring a high quality of life in accordance with our mission and values.
Key Responsibilities
Facilitate smooth transitions for residents entering assisted living from long-term care or independent living.
Develop and maintain social histories, care plans, and required assessments (e.g., mental status exams, depression scales).
Provide psycho-social interventions and evaluate their effectiveness.
Serve as a resident advocate, addressing concerns and resolving complaints.
Conduct interdisciplinary service plan meetings and coordinate care with other departments.
Educate residents, families, and staff on resident rights and available resources.
