Social Worker – Hospice

What You’ll Do
Provide emotional support, counseling, crisis intervention, and grief care for patients and families
Collaborate with the interdisciplinary team to create personalized, culturally sensitive care plans
Connect families to community resources and guide conversations around advance directives and goals of care

Qualifications
Master’s Degree in Social Work (MSW) and 1+ year healthcare experience (hospice preferred)
