Full-time with benefits; 72 hours in the two week pay period, primarily 8am to 4:30pm, Monday through Friday. Weekend and holiday rotations required.
JOB SUMMARY
The Registered Nurse participates as a member of the hospice Interdisciplinary Group (Physician, Nurse, Social Worker, Chaplain) to provide holistic care to hospice patients and to their families. The nurse initiates the Minimum Standard Plan of Care and completes the Functional Health Pattern Assessment. This assessment, assessments from the other Group members, and input from the patient and family are used to formulate a Comprehensive Interdisciplinary Plan of Care to address problems and meet identified needs and goals of the hospice patient and family.
