*HOME HEALTH CARE SPEECH THERAPIST – SEEKING PER DIEM SPEECH LANGUAGE PATHOLOGIST*
VN Home Health Care is an established home care provider in Santa Clara County. We are dedicated to providing high quality, culturally sensitive home care, assisting patients in reaching their highest potential at home, and in integrating back into the community. We are in need of the right person to join our family style company.
Position: Per diem speech language pathologist
Job Description:
Candidate must be able to:
* Complete an initial assessment of patient to determine home care needs in accordance with physician’s orders, plan of care and Home Care policies and procedures.
* Evaluate/treat speech, language, voice, cognition, and swallowing.
* Regularly re-evaluate patient’s speech therapy needs, making necessary revisions as patient status and needs change.
* Be able to identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
* Be able to coordinate patient care among disciplines.
* Should be knowledgeable in Medicare guidelines for home care.
Job Requirements:
* Hold a master's degree in speech language pathology.
* Current speech language pathologist license in State of California
* BLS certification
* Valid driver’s license/insurance.
* Excellent observation, verbal and written communication skills, problem solving skills
* Be flexible, respect others, and be independent
Pay: $100.00 per hour
