Job Title: Physical Therapist (PT) - Post Acute Care

Location: Cottage Grove Post cute
Job Type: Full-Time
Salary Range: $93,000 - $135,200 / year (or $45.00 - $65.00 / hour depending on experience)- Hourly Position

Key Responsibilities
  • Patient Evaluations: Conduct comprehensive initial assessments, taking into account medical history, functional limitations, and patient goals.
  • Treatment Planning: Develop and execute customized, evidence-based physical therapy programs.
  • Hands-on Care: Utilize manual therapy techniques, therapeutic exercises, and modalities to facilitate recovery.
  • Documentation: Maintain accurate, timely electronic medical records (EMR) in compliance with regulatory standards.
  • Collaboration: Communicate effectively with physicians, family members, and other healthcare team members to coordinate optimal patient care.
  • Education: Instruct patients and caregivers on home exercise programs and injury prevention strategies.

Qualifications & Requirements
  • Education: Graduate of an APTA-accredited physical therapy program (Master's or Doctorate degree).
  • Licensure: Current or eligible State Physical Therapist License.
  • Certification: Current Basic Life Support (BLS) / CPR certification- preferred but not required
  • Skills: Strong clinical reasoning, excellent interpersonal communication, and a team-first mindset. New graduates are welcome to apply and will be paired with a structured clinical mentor.
