Job Description
*Home Health Physical Therapist (Part-Time) | Marin County*
Home health PT, Marin County only. You own your patient schedule, evaluate and treat in the home, and work with a clinical supervisor who returns your calls. We keep therapy caseloads geographically tight across San Rafael, Novato, Mill Valley and the surrounding communities, so your day is patients, not freeway.
*Pay and Benefits*
* $55 to $60 per hour, based on experience
* Mileage reimbursed at $0.76 per mile
* 401(k) with company match
* Part-time with a flexible weekday schedule you set
*What You Will Do*
* PT evaluations and treatment visits in the home
* OASIS collaboration on therapy-only cases
* Home exercise programs and caregiver training
* Discharge planning and collaboration with the RN case manager
* Document in the EMR within 48 hours of each visit
*What We Are Looking For*
* Active California Physical Therapist license
* Home health experience preferred; strong SNF or outpatient PTs will be oriented
* Current CPR certification
* Reliable vehicle, valid driver's license and auto insurance
* Ability to pass a background check
* Bilingual English and Spanish a plus, not required
