Job Description
*Family Achievement Center of Central Minnesota (FAC-CMN)* is seeking a *Physical Therapist* to join our collaborative, multidisciplinary pediatric clinic. *About FAC-CMN*
FAC-CMN provides speech, occupational, and physical therapy services to *infants through teens*, both *in person and via telehealth*, with potential opportunities to collaborate with schools across Minnesota. Our multidisciplinary model allows therapists to grow, collaborate, and use their individual strengths every day.
*Job Responsibilities*
* Provide physical therapy services to pediatric clients
* Conduct evaluations and develop individualized treatment plans
* Collaborate with a multidisciplinary team to support child and family goals
* Maintain timely and accurate documentation
* Engage families and caregivers in the therapy process
* Deliver services in clinic and via telehealth as appropriate
*Qualifications*
* Graduate from accredited Physical Therapy program
* *Minnesota PT Board license required*
* Pediatric experience preferred
* New graduates are welcome to apply!!
