*What You’ll Do*
* Provide high‑quality physical therapy services in home health settings.
* Assess functional abilities and help develop individualized plans of care.
* Document accurately and maintain confidentiality.
* Offer education, consultation, and coordination with families and care teams.
* Participate in performance improvement activities and required meetings.
* Uphold PHHI policies, professional standards, and the WV State Practice Act.
*What We’re Looking For*
* Current West Virginia Physical Therapist license (in good standing).
* Current CPR certification.
* Experience as a Physical Therapist (home health experience a plus).
* Strong critical thinking, judgment, and communication skills.
* Ability to work well with patients, families, and team members.
* Meets all federal program participation requirements.
* Experience with discharge planning and developing comprehensive care plans for diverse patient populations.
* Familiarity with EMR systems for documentation and patient management
* Excellent communication skills to effectively educate patients and coordinate with healthcare teams
