POSITION SUMMARY:
The Licensed Practical Nurse is responsible for providing exceptional care to patients under the supervision of a Registered Nurse Case Manager, in accordance with the Plan of Care.
The LPN is a member of the Interdisciplinary Team/Group (IDT/IDG).
ESSENTIAL DUTIES:
* Provides continuous assessment and monitoring of the physical, emotional and spiritual needs of the patient.
* Coordinates patient care with the physician and all members of the Interdisciplinary Team ( RNCM, OT, PT, MSW, ST, Dietitian or Clinical/Nursing Supervisor) through ongoing communication and appraisal of patient’s changing condition. Refers to the appropriate team member for assistance as needed.
* Ensures the educational needs of the patient and family/caregiver are met to promote optimal care.
* Performs complex telephone interviews and triages as appropriate.
* Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
* Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
* Provide direct care to patients as prescribed in the Interdisciplinary Plan of Care to maintain the highest level of comfort and quality of life assuming primary responsibility for case management.
* Attend and participate in patient care conference as needed.
* Investigate and follow through on unusual orders or requests for service or information.
* Participate in the agency’s on-call rotation as prescribed by the needs of the agency to provide nursing service to clints when required outside office hours.
* Other duties and responsibilities as assigned.
POSITION QUALIFICATIONS:
* Minimum Experience: 1-2 years of experience in hospice/palliative care, medical surgical, public health, oncology, acute care or home health.
* Graduate of an accredited practical nurse or vocational nursing program, currently licensed as an LPN in the State of Colorado
* Valid Colorado state driver’s license
* Proof of auto insurance.
* CPR certification
PHYSICAL REQUIREMENTS:
* Ability to lift/push/pull minimum of 50 lbs
* Lifts/positions/ and/or transfers patients
* Lifts supplies/equipment
* Prolonged, extensive or considerable standing/walking/sitting
* Considerable reaching/stooping/bending/kneeling/crouching
* Visual and auditory acuity
Job Type: Full-time
Pay: $25-30/hr
