Occupational Therapist (OTR) to join our established
in-house rehabilitation team . This is an opportunity to work in a high-volume, resource-rich clinical environment where therapists have the space, resources, and support to provide exceptional patient care.

one of the largest rehabilitation gyms in the Bay Area , offering an impressive environment for functional recovery, clinical variety, and professional growth.
Position Details
- Position: Occupational Therapist (OTR)
- Location: White Blossom Care Center - San Jose, CA
- Pay Rate: $50-$65 per hour, DOE
- Schedule: Full-Time, Part-Time, or PRN
- Program: Stable, In-House Therapy Team

Key Responsibilities
- Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans based on physician orders and patient needs.
- Provide evidence-based occupational therapy interventions focused on improving functional mobility, independence, safety, and quality of life.
- Supervise and support Certified Occupational Therapy Assistants (COTAs) in accordance with California regulations and applicable standards of practice.
- Participate in interdisciplinary care conferences, utilization reviews, and rehabilitation meetings.
- Complete timely and accurate evaluations, daily documentation, progress notes, and recertifications.
- Assess patients for appropriate durable medical equipment (DME) and provide training to promote safety and independence.
- Educate patients, families, caregivers, and nursing staff on therapeutic strategies and carryover programs.
- Participate in discharge planning and caregiver training to support successful transitions of care.

Qualifications
- Current and valid California Occupational Therapist (OT) license in good standing.
- Experience in post-acute, skilled nursing, or long-term care is preferred.
- New graduates are welcome and encouraged to apply.

- Excellent communication, teamwork, and organizational skills.
