$45.00 - $55.00 hourly

We are a *small, home-like skilled nursing facility in New Philadelphia, Ohio*, and we're looking for a compassionate and experienced *Physical Therapist to join our team on a PRN basis.*

WHAT YOU'LL DO

As a Physical Therapist, you will be responsible for providing skilled physical therapy services to residents in accordance with professional standards, applicable regulations, and individual resident needs.

Responsibilities include:

* Complete physical therapy evaluations and re-evaluations.
* Develop individualized plans of care based on resident needs and clinical findings.
* Provide skilled physical therapy interventions.
* Establish appropriate functional and rehabilitation goals.
* Provide therapeutic exercise, gait training, balance training, transfer training, mobility training, and other skilled interventions as appropriate.
* Monitor resident response to treatment and modify the plan of care as clinically appropriate.
* Assess changes in resident condition and communicate concerns to appropriate members of the interdisciplinary team.
* Collaborate with nursing, therapy, physicians, residents, families, and other healthcare professionals.
* Participate in care planning and interdisciplinary meetings as appropriate.
* Provide education to residents, families, caregivers, and facility staff.
* Complete accurate and timely documentation.
* Assist with discharge planning and transition-of-care recommendations.
* Promote resident safety, independence, and highest practicable level of functioning.
* Follow all applicable federal and Ohio requirements, professional standards, payer requirements, and facility policies.

QUALIFICATIONSRequired:

* Graduate of an accredited Physical Therapy program.
* Current, active Ohio Physical Therapist license.
* Current CPR certification or ability to obtain according to facility requirements.
* Ability to meet facility employment and health requirements.

Preferred:

* Skilled nursing or long-term care experience.
* Geriatric rehabilitation experience.
* Experience with Medicare/Medicaid rehabilitation services.
* Experience working with medically complex residents.
