*POSITION SUMMARY:*

The RN Case Manager coordinates and provides care for hospice patients and their families. Working closely with physicians and the interdisciplinary team, the RN Case Manager is responsible for ongoing assessment, symptom management, care planning, education, and coordination of services.

*ESSENTIAL DUTIES:*

* Provide skilled nursing assessment, symptom management, education, and support to hospice patients and their families.
* Develop and coordinate individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
* Monitor changes in condition and respond appropriately to patient and family needs.
* Coordinate care and communicate closely with physicians and members of the interdisciplinary team.
* Educate and support patients and caregivers throughout hospice care.
* Participate in interdisciplinary team meetings and ongoing care planning.
* Supervise hospice aides in accordance with agency policy and applicable requirements.
* Complete accurate and timely clinical documentation.

*POSITION QUALIFICATIONS:*

* Required License: Current Colorado RN license. Valid Colorado state driver’s license and proof of auto insurance.
* Minimum Education: Graduate of an accredited school of nursing
* Minimum Experience: 1-2 years of experience in hospice/palliative care or home health.
* CPR certification

*PHYSICAL REQUIREMENTS:*

Physical Requirements: Ability to perform the physical functions necessary to provide nursing care in home and community settings, including assisting with patient positioning and transfers and lifting up to 50 pounds.

Pay: $77,000.00 - $95,000.00 per year
