Position Summary
We are seeking a compassionate, motivated, and skilled *Physical Therapist (PT)* to join our team at Maple Springs of Brigham City in our Skilled Nursing Facility. The Physical Therapist will provide comprehensive evaluation and treatment to residents with a variety of medical, orthopedic, neurological, and age-related conditions. The ideal candidate will be committed to improving residents’ mobility, independence, safety, and overall quality of life.
Essential Duties and Responsibilities
* Evaluate residents’ physical abilities, mobility, strength, balance, coordination, and functional limitations.
* Develop individualized treatment plans based on each resident’s needs, goals, and medical condition.
* Provide skilled physical therapy interventions, including therapeutic exercises, gait training, balance activities, transfers, strengthening, functional mobility training and justify in documentation accordingly.
* Assist residents in maximizing independence with activities of daily living and mobility.
* Monitor resident progress and modify treatment plans as appropriate.
* Educate residents, families, and caregivers regarding therapeutic exercises, safety precautions, positioning, mobility, and home exercise programs.
* Collaborate with nursing staff, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
* Complete accurate and timely evaluations, progress notes, treatment documentation, discharge summaries, and other required clinical documentation.
* Participate in care plan meetings and contribute to the development and implementation of individualized resident care plans.
* Follow all facility policies, professional standards, Medicare/insurance requirements, and applicable federal and state regulations.
* Maintain resident confidentiality and uphold HIPAA requirements.
* Promote resident dignity, safety, comfort, and quality of care at all times.
* Participate in fall-prevention and restorative care programs as appropriate.
* Maintain professional knowledge through continuing education and ongoing clinical development.
* Ability to navigate the NetHealth EMR documentation system; previous experience preferred. Qualifications
* Graduate of an accredited Physical Therapy program.
Physical Requirements
* Ability to safely assist and transfer residents as required.
* Ability to stand, walk, bend, stoop, kneel, and perform physical activities associated with patient care.
* Ability to safely demonstrate therapeutic exercises and mobility techniques.
* Current, unrestricted Physical Therapist license in the state of Utah.
* Current CPR/BLS certification, or ability to obtain certification as required.
* Previous experience in a Skilled Nursing Facility, rehabilitation, hospital, or long-term care setting preferred.
* Strong clinical assessment, communication, organization, and documentation skills.
* Ability to work effectively as part of an interdisciplinary healthcare team.
* Compassionate and professional approach to working with older adults and individuals with complex medical needs.
Pay: $42.00 - $48.00 per hour
