Physical Therapist

Dignity Health East Valley Rehabilitation Hospital - Gilbert seeks a licensed Physical Therapist to provide inpatient rehabilitation services to adults recovering from stroke, brain and spinal cord injury, and other complex conditions. In this mission-driven, patient-first setting, you will complete thorough evaluations, develop evidence-based treatment plans, and deliver intensive therapy focused on mobility, balance, strength, and functional independence. You will collaborate closely with an interdisciplinary team, educate patients and families, document in the EMR, support safe discharges, and contribute to a culture of dignity, compassion, and continuous improvement.

Responsibilities

• Perform comprehensive physical therapy evaluations for inpatient rehabilitation patients.
• Develop and implement individualized, evidence-based treatment plans.
• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Collaborate with physicians, nurses, and other therapists on interdisciplinary care plans.
• Educate patients and families on home exercise programs, safety, and recovery expectations.
• Document evaluations, daily treatments, and progress notes in the EMR accurately and timely.
• Participate in team conferences, discharge planning, and care coordination.
• Contribute to a culture of safety, respect, and continuous quality improvement.
