Peninsula Post Acute is a premier short-term rehabilitation and skilled nursing facility dedicated to helping residents recover, regain independence, and return to their highest level of function. We are seeking a compassionate and motivated Physical Therapist (PT) to join our collaborative in-house rehabilitation team on a PRN basis.

Key Responsibilities
  • Evaluate residents and develop individualized physical therapy treatment plans.
  • Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and overall functional independence.
  • Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes.
  • Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
  • Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to coordinate high-quality patient care.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.
  • Participate in interdisciplinary care conferences and discharge planning.
  • Educate residents and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.


Qualifications
  • Current California Physical Therapist (PT) license in good standing.
  • Strong communication, clinical reasoning, and teamwork skills.
  • Passion for delivering high-quality rehabilitation services.
