Position SummaryWe are seeking a compassionate and motivated
Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, you will provide skilled therapy services that help residents improve mobility, strength, function, and independence.
Key Responsibilities- Provide physical therapy treatments as directed by the supervising Physical Therapist
- Implement individualized treatment plans based on physician orders and therapy goals
- Monitor resident progress and communicate updates to the Physical Therapist
- Document treatment sessions, progress notes, and resident outcomes accurately and timely
- Assist with discharge planning and caregiver education
- Collaborate with nursing staff and interdisciplinary team members to support positive resident outcomes
- Participate in care conferences and team meetings as needed
- Maintain a safe, clean, and organized therapy environment
- Ensure compliance with all state, federal, and facility regulations
- Help residents achieve their highest level of function and independence
QualificationsEducation & Experience- Graduate of an accredited Physical Therapist Assistant program
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Experience with PointClickCare (PCC) and Casamba preferred
- Strong communication, documentation, and organizational skills
Licenses & Certifications- Current South Carolina Physical Therapist Assistant (PTA) license in good standing
- Must maintain all required continuing education requirements
- Must remain in good standing with the State Board
