Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, function, and overall independence. You will work collaboratively with an interdisciplinary team to deliver high-quality, patient-centered care.

Key Responsibilities
  • Perform patient evaluations and develop individualized treatment plans
  • Provide therapeutic interventions to improve strength, balance, mobility, and function
  • Monitor patient progress and adjust treatment plans as needed
  • Maintain accurate and timely clinical documentation
  • Communicate with physicians, families, and care team members
  • Participate in care planning, discharge planning, and rehab meetings
  • Supervise PTAs, therapy aides, and students
  • Provide in-service training and education for staff

Qualifications
  • Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)
  • Active, unencumbered Physical Therapist license in South Carolina (required)
  • Experience in a skilled nursing or rehab setting preferred
  • Familiarity with PointClickCare (PCC) and Casamba preferred
  • Strong clinical, leadership, and communication skills
  • Commitment to continuing education and professional development
