Position SummaryAs a
Physical Therapist (PT), you will evaluate and treat residents to improve mobility, function, and overall independence. You will work collaboratively with an interdisciplinary team to deliver high-quality, patient-centered care.
Key Responsibilities- Perform patient evaluations and develop individualized treatment plans
- Provide therapeutic interventions to improve strength, balance, mobility, and function
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Communicate with physicians, families, and care team members
- Participate in care planning, discharge planning, and rehab meetings
- Supervise PTAs, therapy aides, and students
- Provide in-service training and education for staff
Qualifications- Degree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)
- Active, unencumbered Physical Therapist license in South Carolina (required)
- Experience in a skilled nursing or rehab setting preferred
- Familiarity with PointClickCare (PCC) and Casamba preferred
- Strong clinical, leadership, and communication skills
- Commitment to continuing education and professional development
