Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Experience providing evaluation and plan of care development for geriatric patients preferred.
• Ability to manage caseloads typical of a skilled nursing facility and to document in electronic medical records preferred.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Days 08:30–16:30.
• Schedule: 5 shifts per week, 8 hours per shift.
• Assignment Length: 13 weeks.
• Guaranteed Hours: 35 hours per week.
