Pay Range: $57-$67 per hour
Position Summary
Sunnyvale Gardens Post Acute is seeking a PRN Physical Therapist to join our skilled nursing and post-acute care team. The ideal candidate is compassionate, clinically strong, and committed to helping patients restore function, mobility, and quality of life.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercise, gait training, balance training, and functional mobility interventions
- Document evaluations, treatments, goals, and progress in accordance with regulatory standards
- Collaborate with an interdisciplinary care team to optimize patient outcomes
- Educate patients and families on treatment plans, safety, and home exercise programs
- Maintain compliance with state, federal, and facility policies
Qualifications
- Current California Physical Therapist license in good standing
- Graduate of an accredited Physical Therapy program
- Experience in skilled nursing or post-acute care preferred
- Strong communication, clinical, and documentation skills
- Ability to work independently and as part of a team
Schedule & Compensation
- PRN / Per Diem (flexible scheduling)
- $57-$67 per hour, based on experience
