Position SummaryTice Valley Post Acute is seeking a skilled and compassionate PRN Physical Therapist to join our rehabilitation team. The ideal candidate will provide high-quality, patient-centered care to help residents regain mobility, improve functional independence, and enhance overall quality of life.
Key Responsibilities- Evaluate residents and develop individualized treatment plans based on clinical findings
- Implement therapeutic interventions to improve strength, mobility, balance, and coordination
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
- Educate patients and families on treatment plans, exercises, and safety strategies
- Maintain accurate and timely documentation in compliance with regulatory standards
- Ensure adherence to all facility policies and professional guidelines
Qualifications- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Current California Physical Therapist license (in good standing)
- Strong clinical assessment and treatment skills
- Excellent communication and interpersonal abilities
- Experience in skilled nursing or post-acute care preferred, but not required
