At *Salem Transitional Care of Cascadia Healthcare*, we’re dedicated to helping residents regain independence, strength, and confidence through exceptional rehabilitation and skilled nursing care. We’re seeking a passionate and dedicated *Physical Therapist (PT)* to join our team on a PRN basis.

*About the Role*
As a Physical Therapist, you’ll play a vital role in assessing, planning, and providing physical therapy services to residents with a variety of needs. You’ll develop individualized treatment plans that promote mobility, relieve pain, and improve overall function—all while fostering a supportive, encouraging environment that empowers residents to reach their goals.

*Key Responsibilities*

* Evaluate residents’ functional abilities and create individualized treatment plans in collaboration with physicians and interdisciplinary team members.
* Provide hands-on therapy to improve strength, endurance, balance, and mobility.
* Modify treatment plans based on resident progress and tolerance.
* Educate residents, families, and caregivers on exercises and strategies to support continued recovery.
* Document resident status, progress, and outcomes in accordance with state and federal regulations.
* Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as needed.
* Participate in care planning meetings and contribute to quality improvement initiatives.

*Qualifications*

* Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
* Current Oregon Physical Therapist license.
* Current CPR certification.
* Experience in a skilled nursing, post-acute, or long-term care setting preferred.
* Strong clinical reasoning, documentation, and interpersonal skills.
