*Job Overview*
We are seeking a compassionate and dedicated Home Health RN Case Manager to join our team. This role is essential in providing high-quality patient care to individuals in home health setting. The ideal candidate will have a strong background in nursing, with experience in critical care, geriatrics, and case management. As a RN Case Manager, you will be responsible for coordinating patient care, conducting assessments, and ensuring that patients and their families receive the support they need during this critical time.

*Responsibilities*

* Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
* Coordinate with interdisciplinary teams to ensure seamless delivery of services.
* Provide direct patient care, including monitoring vital signs and managing symptoms.
* Facilitate discharge planning and transition of care for patients moving between settings.
* Maintain accurate medical records and documentation in compliance with HIPAA regulations.
* Educate patients and families about disease processes, treatment options, and end-of-life care.
* Advocate for patients’ needs and preferences throughout their hospice journey.
* Participate in case management activities, including utilization management and diagnostic evaluations.
* Collaborate with primary care providers to ensure continuity of care.
* Stay informed about current medical terminology and best practices in hospice care.

*Qualifications*

* Registered Nurse (RN) license required; Bachelor’s degree in Nursing preferred.
* Previous experience in critical care, ICU, emergency medicine, or geriatrics is highly desirable.
* Familiarity with medical-surgical nursing, behavioral health, pediatrics, or outpatient clinic experience is a plus.
* Strong knowledge of physiology and medical terminology essential for effective patient assessment.
* Experience with discharge planning and post-acute care management preferred.
* Excellent communication skills to effectively interact with patients, families, and healthcare teams.
* Ability to manage multiple priorities while maintaining attention to detail.
