*Key Responsibilities*
* Complete initial evaluations and develop individualized plans of care
* Deliver skilled physical therapy to patients in their homes
* Educate patients and caregivers on mobility, safety, and therapeutic exercises
* Provide accurate and timely documentation
* Communicate changes in patient status to physicians and care teams
* Participate in interdisciplinary case conferences
* Supervise Physical Therapy Assistants (PTAs) when required
* Oversee home health aides when PT is the primary skilled service
*Qualifications*
* Active California Physical Therapist (PT) license
* Graduate of an accredited PT or DPT program
* Valid driver’s license and reliable, insured vehicle
* Current CPR certification
* One to two years of experience preferred (home health experience beneficial but not required)
* Strong communication, organizational, and clinical decision-making skills
* Ability to work independently in a field-based environment
